| To pick up your child from the slopes - or have a friend
do it for you, complete this form and give it to your child's
bus chaperone in the morning. For obvious safety reasons,
if we do not have this completed, signed form, we cannot
release your child to anyone. The adult picking up your
child will need to present photo identification at the Chaperone Hut
(3rd floor of Ski Patrol Building) at Summit Central. |

A season pass is cheaper than 6 daily lift tickets and much easier!
First complete and pay for your Bellevue Ski School registration! Only
registered Bellevue Ski School students are eligible.
WARNING: Ineligible applicants will have their passes
deactivated and cancelled by the Summit with NO REFUND.
You may purchase the season pass ONLINE (New!) or BY MAIL. Students need
only purchase the L-T-D Pass. Note that Ski & Bag Check is available for
an additional $30 (good for HS and MS students). Our instruction provider
is Ullr so you will see that name mentionned as the registered ski school.

To purchase BY MAIL:
Download and print the application. Complete the form and attach a copy of your
2012 Bellevue Ski School registration confirmation sheet as proof of enrollment.
IMPORTANT! Both Student AND Parent must sign the first page of the
Big S Season Pass Application.
Mail completed application by DECEMBER 15 to The Summit at Snoqualmie
or hand deliver to Summit West Season Pass office.
